Community-Based Heart Health Initiatives

This educational webinar focused on engaging community-based organizations and public health departments to meet the goals of the national Million Hearts® Initiative to save one million lives from heart attack and stroke by 2017. Attendees learned what others around the country have done to increase engagement and meet this objective. The webinar features a number of captivating speakers. First, Health Care Excel, the Kentucky Quality Improvement Organization (QIO) shares the “Bless Your Heart” campaign. The campaign consists of a toolkit, originally developed by the Arkansas QIO, created for faith-based organizations to adapt for their use in educating and engaging members in living a heart-healthy lifestyle. Next, Kansas Department of Health and Environment (KDHE), Bureau of Health Promotion Director, Paula Clayton, shares an overview of their interest in Million Hearts® and provides a great example of Million Hearts® intervention in practice, featuring a Kansas Quality of Care safety net clinic. Lastly, Jane Shirley, KDHE’s Local Health Director in the Bureau of Community Health Systems, highlights the promotion of medical homes as well as population health initiatives, such as Million Hearts®, which create a strong interface between primary and public health in Kansas.
