STAR-Setting Targets-Achieving Results
Using the Setting Targets - Achieving Results (STAR) website, home health agencies can set and monitor outcome targets for Plans of Action (POAs) developed for the Publicly Reported Outcome Quality Measures (see listing below.) Registered agencies can view their Quality Measure scores, select appropriate targets and track their improvement progress over a period of time.

The home health agency publicly reported quality measure data on the Home Health STAR site are the data publicly reported by the Centers for Medicare & Medicaid Services (CMS) on Home Health Compare
. The quality measures are derived from the OASIS (Outcome and Assessment Information Set) data set and are updated quarterly. The publicly reported data, which are available to both registered and unregistered visitors to the STAR site, are rounded to the nearest whole number (for example, 10%). In contrast, registered home health agencies can view more detailed quality measure scores to 1 decimal place precision (for example, 10.3%).
