Integrating Care for Populations and Communities
Integrating Care for Populations and Communities (ICPC) is a new initiative launched by the Centers for Medicare & Medicaid Services. This project, also known as Care Transitions, is being facilitated by the Kansas Foundation for Medical Care, Inc. (KFMC), the Medicare Quality Improvement Organization (QIO) in Kansas. The National Coordinating Center for the ICPC aim is assisting QIOs nationwide in conducting this initiative.
The focus of this project is to improve the quality of care for Medicare beneficiaries who transition across healthcare settings, and to reduce avoidable hospital readmissions. KFMC is bringing together Kansas hospitals, nursing homes, rehabilitation facilities, home health agencies, hospices, physician offices, patient advocacy organizations and other stakeholders to form community coalitions to work on this initiative. The goal is to reduce Medicare beneficiary readmissions, within 30 days of hospitalization, by 20% over three years.
KFMC’s role is to assist and support communities in their efforts to adopt wide-spread improved practices to reduce hospital readmissions. Technical assistance and resources are available including:
- Convening a community and building a coalition
- Identifying key drivers of readmissions through a root cause analysis (RCA)
- Determining the target population for interventions
- Selecting evidence-based interventions
- Developing intervention plans
- Helping with the Community-Based Care Transitions Program (CCTP) application
- Providing ongoing measurement data.
