Community Based Care Transitions Program (CCTP)
The Community-based Care Transitions Program (CCTP) is mandated by section 3026 of the Affordable Care Act. The program provides funding to test models for improving care transitions for high-risk Medicare beneficiaries, by using services to manage patients’ transitions effectively. CCTP is part of Partnership for Patients, a national patient safety initiative. One of their goals is to decrease preventable complications during a transition from one care setting to another, reducing hospital readmissions 20 percent by 2013.
The goals of the CCTP are to:
- Improve transitions of beneficiaries from the inpatient hospital setting to other care settings;
- Improve quality of care;
- Reduce readmissions for high-risk beneficiaries; and
- Document measureable savings to the Medicare program.
