Fringe Benefits Package Summary

Health Coverage & Dental Coverage

This program combines basic benefits with additional comprehensive benefits to give you coverage for a wide range of services. With this program, the first dollars of healthcare expense are shared 50/50 until your share reaches a deductible option package ranging from $750 to $3,000 for a single membership and $1,500 to $6,000 for a family membership. Then the Plan will pay 100% of the allowable charge for covered services until the beginning of a new benefit period or until the maximum lifetime limit has been reached. Dental expenses are covered at 100% for basic services and periodontics, and 50% for supplemental and prosthodontics.

Prescription Drug Coverage

Coverage is provided with a copay rate of $15 for generic, $30 for formulary and $45 for non-formularly outpatient prescription drugs which are approved for use in the United States by the Federal Food and Drug Administration and require a written prescription order from the employee's doctor.

Flexible Spending Account

Sometimes known as a Cafeteria Plan

A Flexible Spending Account (FSA) lets employees set aside a certain amount of each paycheck into an account before paying income taxes. During the year, employees are reimbursed from this account for expenses such as healthcare and day care.

KFMC Pension and 401K

Employees are elgible, after 90 days, to participate in the pension program.

Life Insurance and Disability Insurance

KFMC provides life, accidental death and dismemberment and long-term disability insurance to employees.

Personal Days

Employees are eligible for two personal days per year.

Sick Leave

Sick leave is credited to you at the rate of eight (8) hours per month worked for each full time employee.
